The Campaign Finance Handbook will provide information to help you comply with the campaign financial disclosure laws and requirements.

Disclosure Form (EFS Paper Version)

CF-01 - Campaign Financial Disclosure Report (pdf pdf 1.3MB)

This form is no longer used by filers to submit itemized campaign financial disclosure reports, as all filers are now required to submit reports electronically via the EFS Web Application, unless a waiver has been granted by the NYSBOE (EL 14-102(4)).

Certain candidates/committees may still use this form to file In-Lieu-Of-Statements with the New York State Board of Elections (NYSBOE).

Registration Forms

A committee must file the appropriate registration forms to register with NYSBOE and obtain a Filer ID#. Forms must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

Complete the form(s), print and sign. All forms are fillable; however, original signatures are required. We cannot accept any kind of digital or Adobe-generated signatures. Then, mail the form to NYSBOE.

CF-02, Type 1 - Authorized Single Candidate Committee Campaign Finance Registration Form (pdf pdf 162KB)

An Authorized Single Candidate Committee must file this form to register with NYSBOE and obtain a Filer ID#. It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

CF-04 Candidate Registration And/Or To Request NYSBOE Filer ID# Campaign Finance Form (pdf pdf 238KB)

A candidate must file this form to register with NYSBOE and to obtain a Filer ID# if they do not have a registered authorized committee disclosing all of the financial activity of the candidate or the candidate has a registered authorized committee but chooses to have financial activity for the campaign that will not be disclosed by the authorized committee.

CF-02, Type 2 - Political Action Committee (PAC) Campaign Finance Registration Form (pdf pdf 174KB)

A Political Action Committee (PAC) must file this form to register with NYSBOE and obtain a Filer ID#. It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

CF-02, Type 3-7; 3H-7H - Constituted/Party Committees and Housekeeping Campaign Finance Registration Form (pdf pdf 299KB)

A Constituted/Party Committee must file this form to register with NYSBOE and obtain a Filer ID#. It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

CF-02, Type 8 - Independent Expenditure Committee Campaign Finance Registration Form (pdf pdf 356KB)

An Independent Expenditure Committee must file this form to register with NYSBOE and obtain a Filer ID#. It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

An Authorized Multi-Candidate Committee must file this form to register with NYSBOE and obtain a Filer ID#. It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.

Claim of Exemption from Filing

CF-05 - Candidate or Committee Claim of Exemption from Filing Campaign Financial Disclosure Reports (pdf pdf 336KB)

A candidate or committee must file this form with the New York State Board of Elections to claim an exemption from filing.

Authorization Forms

CF-16 - Candidate Authorization for a Committee to Make ALL Campaign Financial Disclosures (pdf pdf 267KB)

File this form if you are a candidate with an authorized committee that will make all of the candidate's required campaign financial disclosure filings. These filings would include all the financial activity of the campaign, including the financial activity of the candidate.

CF-03 - Committee Authorization Campaign Finance Form (pdf pdf 116KB)

All committees that are taking part in the campaign of any candidate by making direct expenditures on the candidate's behalf must file this form.

Complete this form, print and sign it. All forms are fillable; however, original signatures are required. We cannot accept any kind of digital or Adobe-generated signatures. Then, mail the form to NYSBOE.

CF-07 - Authorized User Designation/Removal for EFS Web Application (pdf pdf 189KB)

A treasurer may file this form to designate additional authorized users who can access the filer’s EFS Web Application account. The authorized user will be able to log into the committee’s account, create report drafts, and add/edit/delete transactions. The authorized user will not be able to submit reports. Submitting reports remains the sole responsibility of the treasurer of the committee. Each authorized user will have a different email address and password log-in than the treasurer has.

If a treasurer wishes to remove someone previously designated as an authorized user, the treasurer must submit a new version of this form to NYSBOE.

If the treasurer on the form successfully resigns from the committee, all authorized users designated by that treasurer will automatically lose access to the committee’s EFS Web Application.

Resignation/Termination/Non-Participation Forms

CF-18 - Termination or Resignation Request Form/No-Activity Report Form (pdf pdf 491KB)

A candidate or committee treasurer, which is registered with the New York State Board of Elections (NYSBOE), must file this form requesting resignation as treasurer or termination of the candidate's/committee's filing obligation.

Complete this form, print and sign it. All forms are fillable; however, original signatures are required. We cannot accept any kind of digital or Adobe-generated signature. Then mail the form to NYSBOE.

Please note that a letter of resignation is required when requesting resignation as treasurer.

CF-19 - Application for Electronic Filing Exemption (pdf pdf 150KB)

The committee, which is registered with the New York State Board of Elections (NYSBOE), must file this form when applying for exemption from the electronic campaign financial disclosure reporting requirements of Article 14 of the NYS Election Law.

CF-20 - Non-Participation in Election(s) by Registered PACs, Party and Constituted Committees or Independent Expenditure Committees (pdf pdf 306KB)

The committee, which is registered with the New York State Board of Elections (NYSBOE), must file this form when it will not support or has not supported, in any way, any candidate or political committee which is a participant in the current year's Primary and/or General Election.

Statement of Identity Form

Statement of Identity Form for LLCs/PLLCs (pdf pdf 184KB)

Limited Liability Companies (LLCs), including Professional Limited Liability Companies (PLLCs), that make contributions to or expenditures for candidates or political committees must file a Statement of Identity disclosing all direct and indirect owners of the membership interests in the LLC/PLLC and the proportion of each direct or indirect member’s ownership interest in the LLC/PLLC.

This form must be filed by December 31st of any year in which the LLC/PLLC makes a contribution to or expenditure for a candidate or political committee. The requirement applies to contributions and expenditures made on or after January 31, 2019.
