Resignation and Termination Form

CF-18 - Termination or Resignation Request Form/No-Activity Report Form (pdf 491KB)

A candidate or committee treasurer, which is registered with the New York State Board of Elections (NYSBOE), must file this form requesting resignation as treasurer or termination of the candidate's/committee's filing obligation. 

Complete this form, print and sign it. All forms are fillable; however, original signatures are required. We cannot accept any kind of digital or Adobe-generated signature. Then mail the form to NYSBOE. 

Please note that a letter of resignation is required when requesting resignation as treasurer.

No-Activity Report

CF-18 - Termination or Resignation Request Form/No-Activity Report Form

In order to qualify to file a No-Activity Report, there cannot have been any activity (i.e., receipts and/or expenditures, such as interest, dividends and bank charges) during the reporting period and, therefore, there are no transactions to report. The filer must also have no outstanding loans or liabilities to disclose.

 

A No-Activity Report can be easily submitted electronically via the EFS Web Application, but may be accepted via a paper CF-18 Termination or Resignation Request Form/No-Activity Form.

Non-Participation Form

CF-20 - Non-Participation in Election(s) by Registered PACs, Party and Constituted Committees or Independent Expenditure Committees ( pdf 306KB)

The committee, which is registered with the New York State Board of Elections (NYSBOE), must file this form when it will not support or has not supported, in any way, any candidate or political committee which is a participant in the current year's Primary and/or General Election. 

Complete the form, print and sign it. All forms are fillable; however, original signatures are required. We cannot accept any kind of digital or Adobe-generated signatures. Then, mail the form to NYSBOE.

Claim of Exemption from Filing

Candidates or committees may file a Candidate or Committee Claim of Exemption from Filing Campaign Financial Disclosure Reports form (CF-05) if one of the following is true. (EL 14-124):

  • Candidates have not/will not receive or spend more than $50 for their campaign, including their own personal funds (this includes a candidate for member of a county committee of a political party or any candidate for delegate or alternate delegate to a judicial district convention that will not spend more than $50);
  • Candidates for public office in a town, city or village having a population under 10,000, where the candidate does not raise or spend in excess of $1,000 in the aggregate on the campaign. This includes the personal funds of the candidate;
  • Ballot issue committees formed solely to support or oppose a ballot proposition and have not/will not receive or spend over $100 relative to the ballot or committees solely supporting or opposing a ballot proposition in a town, city, or village having a population under 10,000, where the committee does not raise or spend in excess of $1,000 in the aggregate on the campaign.
     

Note: If after submission of this form the basis for a claim of exemption becomes invalid due to a change in circumstance (e.g., exceeding monetary threshold or scope of candidate/committee activity requires filing), the candidate/committee must register and file all applicable election reports.

Disclosure Form - EFS Paper Version

CF-01 - Campaign Financial Disclosure Report ( pdf 1.3MB)

This form is no longer used by filers to submit itemized campaign financial disclosure reports, as all filers are now required to submit reports electronically via the EFS Web Application, unless a waiver has been granted by the NYSBOE (EL 14-102(4)).
 

Certain candidates/committees may still use this form to file In-Lieu-Of-Statements with the New York State Board of Elections (NYSBOE).
 

In order to qualify to file an In-Lieu-Of Statement, you must be a candidate and/or an authorized committee solely supporting one candidate or a committee involved solely in promoting the success or defeat of a ballot proposal, and at the close of the applicable reporting period, neither the total receipts nor the total expenditures of the campaign have exceeded $1,000. If you have previously filed an In-Lieu-Of Statement and find that you now exceed this $1,000 threshold, you must file an itemized report covering all transactions since the beginning of the campaign. Once an itemized report is required, you may not file an In-Lieu-Of Statement for any future reporting period.

Statement of Identity Form

Statement of Identity Form for LLCs/PLLCs ( pdf 184KB)

Limited Liability Companies (LLCs), including Professional Limited Liability Companies (PLLCs), that make contributions to or expenditures for candidates or political committees must file a Statement of Identity disclosing all direct and indirect owners of the membership interests in the LLC/PLLC and the proportion of each direct or indirect member’s ownership interest in the LLC/PLLC.
 

This form must be filed by December 31st of any year in which the LLC/PLLC makes a contribution to or expenditure for a candidate or political committee. The requirement applies to contributions and expenditures made on or after January 31, 2019.
