The Campaign Finance Handbook will provide information to help you comply with the campaign financial disclosure laws and requirements.
CF-01 Campaign Financial Disclosure Report 01/13 ( 1,006KB)
Candidates/committees use this form to file campaign financial disclosure reports to the New York State Board of Elections (NYSBOE) and/or local Board of Election(s) and when they are not required to file electronically with NYSBOE.
CF-02 Committee Registration/Treasurer and Bank Information 03/09 ( 242KB) | CF-02 Fill-in Form ( 191KB)
A committee must file this form to register with NYSBOE and obtain a Filer ID# and PIN and to register with their appropriate local Board of Election(s). It must be submitted within five days of choosing a treasurer and depository and prior to receiving or expending any funds.
*If you select the Fill-in Form, once it is completed, you need to print the form and sign it. We cannot accept any kind of digital or Adobe-generated signature. Then, mail the form to NYSBOE and/or county board of elections as applicable.
CF-03 Committee Authorization Status 03/10 ( 80KB) | CF-03 Fill-in Form ( 129KB).
All committees that are taking part in the campaign of any candidate by making direct expenditures on the candidate’s behalf must file this form. It should be filed together with the Committee Registration/Treasurer and Bank Information (CF-02)

CF-04 Candidate Campaign Finance Registration Form to Request NYSBOE Filer ID# and PIN 04/12 ( 61KB) | CF-04 Fill-in Form ( 162KB)
A candidate must file this form to register with NYSBOE and to obtain a Filer ID# and PIN if they do not have a registered authorized committee disclosing all of the financial activity of the candidate or the candidate has a registered authorized committee but chooses to have financial activity for the campaign that will not be disclosed by the authorized committee.

CF-05 Candidate or Committee Claim of Exemption from Filing Campaign Financial Disclosure Reports 03/10 ( 60KB) | CF-05 Fill-in Form ( 170KB)
A candidate or committee must file this form at each appropriate board of elections where the candidate or committee is required to file campaign financial disclosure reports.

CF-16 Candidate's Authorization for a Committee to Make ALL Campaign Financial Disclosures 04/12 ( 81KB) | CF-16 Fill-in Form ( 143KB)
File this form if you are a candidate with an authorized committee that will make all of the candidate’s required campaign financial disclosure filings. These filings would include all the financial activity of the campaign, including the financial activity of the candidate.

CF-18 Termination or Resignation Request Form/No-Activity Report Form 11/12 ( 103KB) | CF-18 Fill-in Form ( 322KB)
A candidate or committee treasurer, which is registered with the New York State Board of Elections (NYSBOE), must file this form requesting resignation as treasurer or termination of the candidate’s/committee’s filing obligation.

CF-19 Application for Electronic Filing Exemption 03/10 ( 81KB) | CF-19 Fill-in Form ( 174KB)
The committee, which is registered with the New York State Board of Elections (NYSBOE), must file this form when applying for exemption from the electronic campaign financial disclosure reporting requirements of Article 14 of the NYS Election Law.

CF-20 Notice of Non-Participation in Election(s) by a Registered PAC, Party or Constituted Committee 03/10 ( 112KB) | CF-20 Fill-in Form ( 153KB)
The committee, which is registered with the New York State Board of Elections (NYSBOE), will not support or has not supported, in any way, any candidate or political committee which is a participant in the current year’s Primary and/or General Election.
